The Challenge: Documentation delays draining focus and slowing care

Documentation is essential, but it often happens after long days filled with home visits, calls, and back to back meetings. Staff wrote notes in several steps and sometimes waited 1 to 3 days before entering a visit into the record. That delay made details harder to recall and slowed case progress. Some assessments took up to 3 hours to write. Many described the weight of having unfinished notes sitting in the back of their mind while trying to stay present with the next person in line. Farsund wanted to see whether an AI documentation tool could ease the pressure of documentation for their healthcare workers.

The Solution: Giving clinicians a head start on every note

Tandem’s AI scribe listened during conversations and produced a draft that the clinician then reviewed and approved. That changed the rhythm of the work. Instead of carrying notes forward or splitting them across several sessions, employees could finish documentation right after the meeting.
